The ] line between Chennai and ] cuts through this highway in many places resulting in huge pile up of vehicles on both sides of the highway, especially during the night hours. In case of any mishaps on the highway, the emergency services get delayed by at least 3 - 5 hours, due to the volume of traffic.

In an effort to circumvent these longstanding problems, many ] projects are being carried out for this highway which includes road over-bridges along the Chennai Meenambakkam airport road and several rail over-bridges over the railway crossings on the ], which can cut travel time as well as reduce traffic. The six-lane system is currently extended between ] and ] for a distance of around 225 km (135 miles).

Grand Southern Trunk Road or National Highway 45 is an arterial road in the Indian state of Tamil Nadu. It is also known as Chennai–Perambalur-Trichy-Dindigul-Theni Highway. The road starts from Kathipara Junction in St Thomas Mount, Chennai towards Perambalur, Trichy, Dindigul and Theni. It branches out from Anna Salai (previously known as Mount Road).

The road runs through important localities like Meenambakkam, Pallavaram, Kadaperi, Tambaram, Vandalur, Kilambakkam, Guduvanchery, Paranur, Chengalpattu, Madurantakam, Tindivanam, Villupuram, Vikravandi, Ulundurpet, Perambalur, Trichy, Dindigul, and Theni.
