Paraphilic infantilism is a paraphilia that involves the desire to wear diapers and be treated as a helpless infant. Those who practice it are called an Adult Babies (ABs). Adult Babies and Diaper Lovers (DLs) are often referred to by the combined acronym, AB/DL. The majority of AB/DLs are heterosexual males . Most AB/DLs realize that they are different between the ages of 5 and 15. At this age, they realize that they have a focused, persistent urge that others don't.

AB/DL is not directly harmful, but there are indirect psychological and social dangers. AB/DLs may have thought that they were only one with these urges, and may grow up with the stigma of being “perverts.” In some, this leads to isolation, guilt, and despair. The social and professional risks of being discovered can be severe. Many AB/DLs have tried desperately to change. Few, if any, are successful.

Adult babies are those infantilists for whom the textbook definitions apply, while for a diaper lover there may not be regression to an infantile state.
