It can be performed by both lesbian and heterosexual couples, and some women with very flexible bodies and wrists can do it by themselves.
Fisting is not as impossible as it first seems to some people. In the first place, it is simply an extension of genital manipulation and follows somewhat naturally from the woman enjoying having her partner masturbate her. An exercise recommended to couples who are preparing to give birth, perinial massage, often results in fisting.
It was exposed to wider public knowledge in a famous (but not X-rated) scene in the Kevin Smith movie "Chasing Amy".
